Each adult applicant is to fully complete a copy of thisform and the completed copies are to be returned to our office with yourresidence application.

Medical and Chest X-ray Certificates(INZ 1007) thesemust be completed by each applicant and submitted with your application.

Health Requirements Leaflet (INZ 1121)

Your medical examinations must be undertaken through adoctor who has been approved by Immigration New Zealand.Page seven ofthe leaflet under the heading “Where do I go for a medical examination” has alink to the list of approved panel doctors.
